What is Valtrex?
Valtrex is an oral prescription medication that is selectively absorbed into cells infected with Herpes Simplex Virus. Within infected cells, Herpes Simplex Virus coded enzymes convert Valtrex into its active form. This active form inhibits the replication and growth of Herpes Simplex Virus by interfering with viral DNA replication.

Since Valtrex is preferentially taken up and converted into its active form only by Herpes Simplex Virus infected cells, Valtrex is much less toxic for uninfected cells.

Valtrex comes in two strengths: 500mg and 1 gram (1000mg) caplets. It is oblong and colored dark blue. The 500mg caplet is smaller than the 1 gram caplet. Both come with an engraving that states "Valtrex 500" and "Valtrex 1 gram" respectively.

About Herpes Simplex Virus
The Herpes family has over 80 known viruses. Of these, 8 are known to cause disease in humans. The most common disease is Herpes Simplex Virus Type-1 (HSV-1) and type-2 (HSV-2). HSV-1 and HSV-2 look the same under the microscope, and either can infect the mouth (oral herpes, lip blisters, mouth blisters) or genitals (genital herpes). Usually, HSV-1 occurs in the oral region, and HSV-2 occurs in the genital region.

Herpes Simplex Virus Type-1 usually causes herpes labialis commonly known as oral herpes, cold sores or fever blisters. These are highly infectious open sores that crust over before healing. Although not as likely, HSV-1 can cause genital herpes.

Herpes Simplex Virus Type-2 is a contagious viral infection primarily causing genital herpes in men and women. The most common signs and symptoms of genital herpes include recurrent clusters of blisters, bumps and rashes in genital areas. HSV-2 can also cause herpes labialis (cold sores), although less often than HSV-1.

Transmission of Herpes Simplex Virus
Unlike other viruses transmitted through air, Herpes Simplex Viruses require physical contact. The most common cause of transmission is mouth-to-mouth (HSV-1) or genital-to-genital (HSV-2) contact. However, there are other possible means of transmission, such as hand-to-genital and mouth-to-genital contact.

What is episodic therapy?
With episodic therapy, you take medication for several days at the first sign of an oral or genital Herpes Simplex Virus outbreak. Episodic therapy disrupts the mechanism by which the herpes virus replicates itself and spreads to other healthy cells. Taking Valtrex early in the reactivation cycle may shorten the length or severity of Herpes Simplex Virus symptoms.

Is there an alternative to episodic therapy?
Yes. If anticipation of your next outbreak is troublesome or you have severe outbreaks often, you can consider suppressive therapy for Herpes Simplex treatment. Suppressive therapy involves taking medication every day to suppress outbreaks before symptoms occur.

If you have few outbreaks, or outbreaks are minor, episodic therapy is probably the better treatment.

How effective is Valtrex for episodic therapy?
In clinical trials, Valtrex shortened the healing time of genital herpes sores by an average of 33% (four vs. six days) compared to placebo. Also, pain duration associated with genital herpes was decreased 25% (three vs. four days) in patients receiving Valtrex versus placebo. In addition, Valtrex stopped viral shedding by an average of 50% (two vs. four days) faster than placebo. Viral shedding occurs when the Herpes Simplex virus is released from the skin during a genital herpes outbreak.

How is Valtrex taken for episodic therapy?
• On occurrence of the first episode of infection with Herpes Simplex Virus take 1 gram of Valtrex twice daily for 10 days.
• For subsequent recurrent episodes of infection with Herpes Simplex Virus take 500mg of Valtrex twice daily for 3 days.

What is suppressive therapy?
With suppressive therapy, medication is taken every day to help suppress outbreaks of Herpes Simplex Virus infection before they occur. While on suppressive therapy, some patients can go 6 months or longer without a herpes outbreak.

How is suppressive therapy different from episodic therapy?
In episodic therapy, medication begins as soon as symptoms of an outbreak occur in order to reduce the severity and duration of an outbreak. Treating genital Herpes Simplex Virus infections this way does not affect the number of outbreaks. In contrast, suppressive therapy involves taking medication daily to help suppress outbreaks before symptoms actually start.

Is suppressive therapy right for me?
If genital herpes recurs with great frequency and severity or affects your confidence and self-esteem, you are a candidate for suppressive therapy for Herpes Simplex treatment.

How effective is Valtrex for suppressive therapy?
• Over 55% of patients experiencing frequent outbreaks (6 or more per year) did not have a single outbreak for 6 months when taking a 1-gram Valtrex Caplet once a day.
• Only 7% of those taking placebo avoided an outbreak for the 6 month period.
• At the end of 12 months, 33% of patients taking Valtrex still had not had a single outbreak. Only 4% of those taking placebo did not have an outbreak.
• Patients with 9 or fewer outbreaks showed comparable results with Valtrex 500mg once daily.
There is no data on the effectiveness or safety of suppressive therapy longer than 1 year.

How is Valtrex taken for suppressive therapy?
• The recommended dosage of Valtrex for chronic suppressive therapy of recurrent genital herpes is 1 gram one time a day.
• For those patients with a history of 9 or fewer recurrent episodes per year, an alternative dosage is 500mg one time a day.

If I am on therapy, can I still spread Herpes to others?
YES, you can still spread the Herpes Simplex virus. No Herpes medication or Herpes treatment has been proven to prevent the spread of herpes infection. You and your partner should always use condoms and avoid sexual contact during a herpes outbreak.

Side effects of Valtrex
For the first several days after beginning Herpes Simplex treatment therapy, you may experience the following side effects for Valtrex:
• nausea
• vomiting
• headache
• loss of appetite
• weakness
• stomach pain
• dizziness

If any of these effects continue or become an annoyance, you may have to stop using Valtrex.
