What form does Tramadol (Ultram) come in?
Ultram is the brand name for Tramadol (generic form). Tramadol is a white, bitter, crystalline, odorless powder. Each Ultram tablet contains 50mg of Tramadol hydrochloride and is white, film-coated capsule-shaped tablet. The tablet is engraved with "McNeil" on one side and "659" on the other.

How does Ultram work?
Ultram is a synthetic analgesic compound (unlike codeine and its derivative medications). Ultram acts on the central nervous system to control pain. Though its mode of action is not completely understood, from animal tests, at least two complementary mechanisms are prevalent: binding of parent compound and its metabolite to opioid receptors and weak inhibition of reuptake of nor epinephrine and serotonin by nerves located within the central nervous system.

Ultram-induced analgesia is only partially reversed by the opiate antagonist naloxone in several animal tests. Ultram demonstrated ability to inhibit reuptake of nor epinephrine and serotonin in vitro. These mechanisms may contribute independently to the overall analgesic profile of Ultram. Analgesic effect in humans begins about one hour after administration and peaks in approximately 2-3 hours.

How effective is Ultram for arthritis treatment?
In clinical studies, Ultram was given to patients in single oral doses of 50, 75, 100, 150 and 200mg. These patients were experiencing pain following surgical procedures and oral surgery (extraction of impacted molars). In single-dose models of pain following oral surgery, pain relief was achieved in some patients at doses of 50mg and 75mg. A 100mg Ultram dose provided analgesia superior to codeine sulfate 60mg, but was not as effective as the combination of aspirin 650mg with codeine phosphate 60mg. In single-dose models of pain following surgical procedures, 150mg provided analgesia generally comparable to the combination of acetaminophen 650mg with propoxyphene napsylate 100mg, with a tendency toward later peak effect.

Ultram pain relief pills have been studied in 3 long-term controlled trials of 820 patients, with 530 receiving Ultram pain relief treatment. Patients with a variety of chronic painful conditions were studied in double-blind trials of 1-3 months. The average daily Ultram dose was approximately 250mg in divided doses comparable with 5 doses of acetaminophen 300mg with codeine phosphate 30mg (Tylenol with Codeine #3) daily, 5 doses of aspirin 325mg with codeine phosphate 30mg daily, or 2-3 doses of acetaminophen 500mg with oxycodone hydrochloride 5mg (Tylox) daily.

Ultram dosage
For the treatment of pain:
Ultram 50mg-100mg can be administered as needed for relief every 4-6 hours, not to exceed 400mg daily.

For moderate pain:
Ultram 50mg may be adequate as the initial dose

For severe pain:
Ultram 100mg is usually more effective as the initial dose.

Dosage considerations
Patients 65-75 years old: No dose adjustment necessary.
Patients over 75 years old: No more then 300mg/day in divided doses
Patients with cirrhosis: Recommended dosage is 50mg every 12 hours.
Patients with kidney impairment: The dosing interval of Ultram should be increased to 12 hours with 200mg maximum daily dose.
Dialysis patients: These patients can receive their regular dose on the day of dialysis.
Patients receiving chronic carbamazepine: Doses up to 800mg daily may be required, this is up to twice the recommended dose of Ultram.

Ultram side effects
Ultram was administered to 550 patients during the double-blind or open-label extension periods in U.S. studies of chronic non-malignant pain. Of these patients, 375 were 65 years old or older. The table below reports the cumulative incidence rate of adverse reactions by 7, 30 and 90 days for the most frequent reactions (5% or more by 7 days).

Most frequently reported events were in the gastrointestinal system and central nervous system. Although reactions listed in the table are felt to be probably related to Ultram administration, the reported rates also include some events that may have been due to underlying disease or concomitant medication usage.

Overall incidence rates of adverse experiences in these trials were similar for Ultram and the active control groups, acetaminophen 300mg with codeine phosphate 30mg, and aspirin 325mg with codeine phosphate 30mg.

Drug abuse and dependence
Ultram has potential to cause psychic and physical dependence of the morphone-type. The drug has been associated with craving, drug-seeking behavior and tolerance development. Such cases of abuse and dependence on Ultram pain relief pills have been reported.

Opioid-dependent patients should not use Ultram since Ultram can re-initiate physical dependence in patients previously dependent or chronically using other opioid drugs. In patients with a tendency to abuse drugs, history of drug dependence or chronic opioid use, Ultram is not recommended.

Ultram and seizures
Seizures have been reported in patients receiving the recommended dosage of Ultram. Concomitant use of Ultram increases seizure risk in patients taking any of the following:
• Selective serotonin re-uptake inhibitors (SSRI antidepressants or anoretics-phentermine)
• Tricyclic antidepressants and other tricyclic compounds (cyclobenzaprine, promethazine, etc.)
• Opioids
• Neuroleptics
• MAO inhibitors
• Any drugs that reduce seizure threshold
• Epilepsy medication

Ultram and drowsiness
Ultram may impair mental or physical abilities needed for performance of potentially hazardous tasks such as operating machinery or driving.

Ultram should not be taken with alcohol.

Ultram should be used with caution when taking tranquilizers, hypnotics or other opiate containing analgesics.

Ultram and pregnancy
• Ultram should not be used in pregnant women or nursing mothers; safe use of Ultram during pregnancy is undetermined.
• Chronic use during pregnancy may lead to physical dependence and post-pregnancy withdrawal symptoms in the newborn child.
• Ultram has been shown to cross the placenta
• The effect of Ultram, if any, on later growth, development, and functional maturation of the child is unknown.
